385 Skin cancers in patients with actinic keratoses

Our study aimed to explore the unknowns regarding absolute and relative risks of future skin cancers in patients with actinic keratoses(AKs). We performed a retrospective cohort using de-identified, random sample 4,999,999 fee-for-service Medicare beneficiaries aged ≥65 years (2009-2018). included treated AKs and, as comparator group, benign lesion seborrheic keratosis(SK). Both groups required at least 1 year between dataset entry first AK or SK diagnosis. excluded history HIV, organ transplant, prior cancer. evaluated any cancer (keratinocyte carcinoma(KC) melanoma) over time AKs. also compared SKs an adjusted competing regression. total 555,945 (53.6%) 481,024 (46.4%) SKs, mean follow-up 5.6(SD 2.8) 4.6 (SD 2.7) years, respectively. Absolute risk after was 6.3% (95% CI 6.3%-6.4%) year, 18.4% 18.3%-18.5%) 3 23.6% 23.5%-23.7%) 4 years. By type, 23.1% 22.9%-23.2%) for KC 1.2% 1.1%-1.2%) melanoma.
